GLP-1 coverage by state
Only 13 state Medicaid programmes cover GLP-1s for obesity. Four dropped coverage in the last year. Here is the national picture and where it changed.

States where coverage has been proposed for removal
Massachusetts and Rhode Island both had live proposals to end Medicaid coverage of GLP-1s for weight loss as of April 2026. Neither had taken effect at the time of reporting. If you are eligible in either state, the sensible read is that the window may not stay open.
States that eliminated coverage
California, Pennsylvania, South Carolina, New Hampshire all ended coverage of GLP-1s for obesity after October 2025, which KFF attributes to budget pressure and drug cost.
States that changed course
North Carolina dropped coverage in October 2025 during a budget stalemate and reinstated it that December — the change that brought the national total back to 13.
Time-limited coverage
Utah reported that funding for coverage was limited to fiscal year 2026 — a deadline rather than a refusal.
The states at the extremes
West Virginia, Mississippi, Colorado stand out on prevalence rather than policy. Every US state and territory recorded adult obesity prevalence of at least 25% in 2024.

Then we have nothing state-specific and verifiable to tell you, and we would rather say so than pad this out. Important. True in every state. Coverage for obesity is optional, but coverage is required for type 2 diabetes, cardiovascular disease (Wegovy, since March 2024) and moderate-to-severe obstructive sleep apnea in adults with obesity (Zepbound, since December 2024). A refusal for weight loss is not a refusal under those indications.
