Why Delaware is different
Delaware is a small programme maintaining coverage while its larger Northeast neighbours cut it — Pennsylvania eliminated coverage, and Massachusetts and Rhode Island have proposals to. Delaware is classified by CDC in the South, and is among the states with adult obesity prevalence at 35% or higher.

What to do in Delaware: Confirm prior-authorisation criteria, and if you are near the Pennsylvania border do not assume regional summaries apply — the two states are now in opposite positions.
Coverage for type 2 diabetes, cardiovascular disease and sleep apnea is mandatory everywhere, so a refusal framed around weight loss is not the end of it — and on Medicare, the $50 programme starts 1 July 2026 regardless of which state you are in.
